Rural Ambulance Crisis
San Bernardino County’s rural ambulance providers — Barstow Desert Ambulance, Baker Ambulance, and Morongo Basin Ambulance — are on the brink. These 911 responders cover massive “super rural” territories, yet face a broken reimbursement system that pays them as little as $339 per run, while public agencies earn over $1,000 for the same service.

The Fix: Super Rural Rate Bonus
We propose that IEHP adopt a Medicare Super Rural Rate Bonus Framework, modeled on the federal program, to bring fairness to rural EMS reimbursement. This would deliver up to $2 million in annual support for these vital providers.

Support Rural Hospitals
Safety-net hospitals in Barstow, Needles, and Twentynine Palms need sustained public investment. These facilities are healthcare anchors for isolated communities and critical to regional emergency care.
Invest in Infrastructure
We also advocate for expanding broadband, water systems, clean energy, and affordable housing across the High Desert to support long-term health and sustainability.
Why This Matters
Rural EMS providers are the only line of defense for thousands of residents across California’s largest and most rugged county. Without this modest funding fix, cutbacks, service delays, and closures are a real threat.
